How do I make a claim?

Step 1: Contact us

Call us on 0800 108 108 (Mon to Fri, 9am to 8pm NZST) or email us at customerservice​@momentumlife.co.nz to let us know you will be making a claim.

Your dedicated Claim Manager will talk you through the claims process and provide you with the necessary forms. You can also download a claim form from the links below:

Step 2: Send us all necessary details

Please complete the forms provided to you and return to us with the necessary documentation by mail.

Step 3: We’ll be in touch

Your dedicated Claim Manager will handle the rest, keeping you updated throughout the process. Once your claim form and required documentation is received, your claim will be assessed and all valid claims will be paid without unnecessary delay.
