Momentum Life $500 Prezzy Card Offer (“Offer”)

 

Receive an obligation free Momentum Life quote for Funeral, Accident or Life Insurance and automatically enter into a prize draw to win a $500 Prezzy Card (“Prezzy Card”). Take out any insurance policy with Momentum Life and automatically be entered into the draw a second time.

There are some special conditions that we would like you to understand about this offer:

1. This promotion commences on the 1 June 2018 and ends at 11.59pm on the 14 December 2018. (“Offer Period”). Momentum Life reserves the right to extend the Offer Period at any time.


2. Participation in the Offer is only open to New Zealand residents: aged 16 years or over; who during the Offer Period get a quote for a new Momentum Life insurance policy and/or take out a Momentum Life Funeral, Accident or Life Insurance Policy.


3. The prize draw will take place on 19 December 2018 at Level 1, 103 Carlton Gore Road, Newmarket, Auckland 1023. Prize: The first name drawn will be the winner of the Prezzy Card. There is only one (1) Prezzy Card to be won.


4. Persons are eligible for up to 2 entries during the Offer Period. One entry when getting a quote and one entry when purchasing a Policy. If multiple quotes are taken out during the Offer Period, only the first quote will qualify as a valid entry. To be eligible for the second entry, your Policy must still be in force at the time of the prize draw.


5. Winner Notification: The winner will be contacted via telephone using the contact details provided when getting the quote. If a winner cannot be contacted within 10 working days, Momentum Life reserves the right to redraw the prize. Once notified, the winner will also be mentioned on the Momentum Life website after the Offer Period.


6. The Prezzy Card is not transferable or exchangeable. If a Prezzy Card or receipt of it incurs a tax liability, the relevant recipient of the Prezzy Card (“Recipient”) is liable for payment of such tax. 7. Momentum Life reserves the right to request the Recipient of the Prezzy Card to provide proof of identity, proof of residency at the nominated delivery address and/or proof of offer eligibility in order to claim the Prezzy Card. Proof of identification, residency and eligibility considered suitable for verification is at the discretion of Momentum Life. In the event that suitable proof cannot be provided, the Prezzy Card Offer will be forfeited in whole and no substitute Offer will be made.


8. Momentum Life reserves the right to disqualify anyone who tampers with the Offer eligibility process, who submits an entry that is not in accordance with these terms and conditions or who has, in Momentum Life’s opinion, engaged in conduct in entering the Offer which is fraudulent, misleading, deceptive, or generally damaging to the goodwill or reputation of Momentum Life.


9. If this Offer is interfered with in any way or is not capable of being conducted as reasonably anticipated due to any reason beyond the reasonable control of Momentum Life, Momentum Life reserves the right, in its sole discretion, to the fullest extent permitted by law, and subject to any written directions from a regulatory authority, to modify, suspend, terminate or cancel the promotion, as appropriate.


10. To the fullest extent permitted by law, in no event will Momentum Life be liable to any person for any claim in tort (including negligence), contract, statute, law or otherwise for any loss, damage, cost or expense (including loss of profits or anticipated savings, economic loss or any special, indirect or consequential damage), incurred by a party or a third party in connection with any act or omission under or in relation to the Offer.
